Benefits

You can safely store your immunization records in ImmTrac at no cost to you. Your records will be kept secure and confidential. Even if you got one vaccine at your doctor’s office and another at your local pharmacy, you can electronically store all your immunization records in ImmTrac. Then, you will have one immunization history.

You can request a copy of your immunization history when you need it for college entrance requirements, military enlistment, travel, employment in health and safety fields, and other instances.

Your ImmTrac authorized health-care provider can access your immunization history and make sure you are not over–or under–vaccinated. This way, you will be fully protected against vaccine-preventable diseases and save money by not paying for vaccines you do not need.

AT NO COST TO YOU. ImmTrac is a service from the State of Texas created by the Department of State Health Services.

SECURE. All the immunization records in ImmTrac are safe and secure. Only ImmTrac authorized health- care providers can access the immunization records.

CONFIDENTIAL. Before anyone can view and release your immunization history, he or she must have your signed authorization. Only a limited number of authorized health-care providers have access to view and release immunization records.

ELECTRONIC. ImmTrac is designed to enable the proper collection and storage of immunization records to make accurate immunization histories.

ACCESSIBLE TO AUTHORIZED HEALTH-CARE PROVIDERS. Authorized ImmTrac health-care providers can access ImmTrac immunization histories with signed authorizations.

About Adult Vaccines

As an adult, you need to stay up to date with your vaccines. There are vaccines now available that may not have been available when you were a child. It is sometimes hard to remember what vaccines you have had. Whether you are getting a flu vaccine each year or getting vaccines for travel, keeping up with your immunization records is important.

INFANTS & CHILDREN (BIRTH – 6 YEARS)

Step 1 - Download

Download and complete the ImmTrac Adult Consent Form (F11-13366) from ImmTracforEveryone.com.

To recieve a link to download the form on your computer send yourself a reminder to register for ImmTrac.

Tips

  1. Please, use a black or blue pen and print clearly.
  2. Click the ImmTrac Adult Consent Form link to open the form.
  3. If you have trouble downloading the ImmTrac Adult Consent Form, please call the Department of State Health Services, 1-800-252-9152.
  4. Please, make sure that you sign the ImmTrac Adult consent form.

Step 2 - Gather and make copies

Gather and make copies of all your official immunization records.

Tips

You may find a copy of your immunization records:

  1. At your last known health-care provider
  2. In your baby book
  3. At the last known elementary, middle or high school attended

Step 3 - Send in

Send in your completed ImmTrac Adult Consent Form and copies of your official immunization records to your authorized health-care provider, local health department (LHD), DSHS health service region (HSR) office or the Department of State Health Services ImmTrac Office.

You can also deliver your consent form by:

  1. Faxing your ImmTrac Adult Consent Form and copies of your official immunization records to ImmTrac at (866) 624-0180.
    Please allow up to 10 business days for production.
  2. Mailing to: Texas Department of State Health Services

Tips

  1. If you are having difficulties sending the ImmTrac Adult Consent Form by fax, please call the Department of State Health Services Immunization Branch at (800) 252-9152, for assistance.
  2. Please, make sure the immunization records sent to ImmTrac were signed by a health-care provider.
